Pengobatan Pilihan Terkini Pengobatan Pilihan Terkini pada Sarkoma Stroma Endometrium Rahim Current Treatment Options in Uterine Endometrial Stromal Sarcoma

Sarkoma uteri adalah bentuk kanker uterus yang jarang. Terjadi pada wanita usia 40-60 tahun yang umumnya ditandai dengan prognosis yang buruk, rekurensi lokal yang tinggi dan metastasis jauh. Endometrial Stromal Sarcoma (ESS) terjadi sekitar 0,2% dari seluruh keganasan ginekologi. Pengobatan yang ditempuh meliputi operasi, radioterapi, kemoterapi dan terapi hormonal. Randomized trials yang menganalisis terapi-terapi tersebut memang terbatas dikarenakan jarangnya kasus ini, oleh karenanya Standar terapi ESS sejauh ini belum ada. Untuk memperlihatkan gambaran mengenai berbagai alternative terapi pada ESS, maka dilakukan penulusuran melalui Medline, Embase dan Cochrane Library. Makalah ini melaporkan kasus seorang wanita usia 32 tahun di RSUP Sardjito, Yogyakarta dengan ESS stadium II. Terapi awal menggunakan tamoxifen dan perfusi lokal cisplatin dengan hasil terjadinya progresifitas penyakit sehingga terapi ini dihentikan. Berikutnya, pendekatan terapeutik dengan menggunakan kombinasi doxorubicin dan ifosfamide sebanyak 2 siklus, yang kemudian dilanjutkan dengan terrapin pembedahan. Lima tahun setelah operasi, pada pasien didapatkan remisi komplit. Oleh karena itu disimpulkan bahwa meski pun belum ada data yang didukung oleh penelitian randomized trials, kemoterapi pada kasus ESS lanjut maupun metastasis dapat memberikan peiuang untuk selanjutnya dilakukan operasi dan memberikan remisi dalam jangka panjang.Warning: file_put_contents(): Only 0 of 23 bytes written, possibly out of free disk space in /var/www/ojs-3.2.1-5/lib/pkp/classes/cache/FileCache.inc.php on line 90
