Penatalaksanaan Perforasi Korona pada Pulpektomi Gigi Decidui

Perforasi iatrogenik merupakan salah satu resiko yang sering terjadi dalam perawatan endodontik yang disebabkan oleh kesalahan operator. Perforasi dapat terjadi pada apeks, lateral atau korona. Perawatan untuk perforasi dapat dilakukan dengan bedah atau non bedah. Prognosis dari perforasi endodonti dipengaruhi oleh beberapa faktor antara lain adalah waktu terjadinya perforasi, lokasi perforasi dan besarnya perforasi. Tujuan laporan kasus ini untuk melaporkan perawatan perforasi korona pada pulpektomi gigi decidui. Kasus seorang anak laki-laki usia 6 tahun datang bersama ibunya dengan keluhan gigi bawah belakang kanan sering sakit tiba-tiba sejak tiga bulan yang lalu. Diagnosis pada gigi molar pertama kanan rahang bawah adalah karies profunda kelas I dengan pulpitis irreversible. Perawatan yang dilakukan adalah pulpektomi. Dalam proses perawatan terjadi perforasi korona di mesial yang disebabkan oleh trauma bur preparasi saat dilakukan pembukaan atap pulpa. Perawatan pulpektomi tetap dilanjutkan dengan menutup daerah perforasi dengan zinc phosphat cement dan diakhiri dengan tumpatan tetap stainless steel crown (SSC). Kesimpulan dari laporan kasus ini yaitu perawatan perforasi korona dengan non bedah pada pulpektomi gigi decidui mempunyai prognosis yang baik. Perawatan dilakukan dengan bahan tumpatan yang memiliki ketahanan terhadap saliva dan tidak mengiritasi pulpa ataupun gingiva.
